Request Accommodations

The following information and forms are only to be used by students who are currently registered with the SDRC. 

Accommodation Information

We recommend that you speak with your professor about your registration with our office and your accommodations before classes begin. 

Purpose of request: Notifies professors of your registration with our office and of your approved accommodations.

  • To complete this: Sign into your AIM account and submit your request for accommodations for each of your classes listed in AIM.

Purpose of request: Request that a test be taken with the SDRC in our proctoring room. Requests must be submitted at least 5 business days in advance.

  • To complete this: Sign into your AIM account and select Alternative Testing in the left-hand toolbar.
  • Follow the steps to complete your request. Late requests may not be accepted due to limited space. 

Purpose of request: A student who is approved for a reduced course load must complete the form for financial aid to recognize you as a full-time student with a lighter course load.

  • It is recommended to submit the request for financial aid at the beginning of the semester.
  • If you are taking a reduced course load of less than 12 hours, you will be required to pay additional fees such as health, athletics or other non-academic fees as appropriate in order to participate in these activities. The Bursar's Office provides additional details about these optional fees.
  • Log in to the Student AIM Portal to submit a reduced course load request.

Purpose of request: Request alternative format for your textbook. This will help ensure your books will be ordered in a timely manner.

  1. You will need: Book title, book edition, publisher, publish date, ISBN number and proof of purchase.
  2. Sign into your SDRC AIM account using the same credentials as for Blackboard.
  3. If you have not requested accommodations for the current semester, please submit those requests first.
  4. Select "Alternative Format" on the left-hand side of your AIM portal and complete the required sections.

Purpose of request: Request a note taker for one or more classes of a student that is provided this accommodation.

  1. Sign into your SDRC AIM account using the same credentials as for Blackboard.
  2. If you have not requested accommodations for the current semester, please submit those requests first.
  3. Select "Notetaking Services" on the left-hand side of your AIM portal and confirm your request for any class in which you need a notetaker. 
  4. As a reminder, notetakers are volunteers and it is not guaranteed that a notetaker will be secured. In that case, assistive technology may be offered as a comparable service. 

Purpose of form: Request a sign language/ASL interpreter and/or captioning for a USC activity

  • You will need: Activity name, the start date, end date, time and location of event, as well as information for a contact person. 
  • The SDRC will work to find an interpreter for your event.
  • Click here to complete the form
  • The SDRC will bill the appropriate party within 30 business days of the event. 

*Please note: this form is not for SDRC registered students. Students registered with the SDRC who need interpreting or captioning for their classes must use the Deaf and Hard of Hearing module on AIM.*

Purpose of form: Used when there is not a wheelchair desk available in a classroom.

Purpose of form: Notify the USC Facilities department if there is an area on campus that makes it very difficult, or impossible, for you to reach your destination (such as a walkway through campus, entrance/exit area of a building, or elevator in a hallway) and there is no alternative route you can take.
